One number for the size of your engine

VO₂ max is the maximum amount of oxygen your body can take up and use during intense exercise. It reflects how well your heart, lungs, blood, and muscles work together to deliver and burn oxygen — in effect, the size of your aerobic engine. [4]

The 122,000-person picture

In a study of 122,007 adults who completed treadmill tests, higher fitness tracked with lower death rates over a median of more than eight years. The benefit kept rising with fitness, with no upper limit observed in the data. [1]

The least-fit group had roughly five times the mortality risk of the fittest. Even among already-fit people, the most elite still did better than the merely high. And low fitness carried a risk on par with or greater than smoking, diabetes, and coronary artery disease in that population. [1]

Fitness as a vital sign

The signal is strong enough that the American Heart Association has argued cardiorespiratory fitness should be assessed routinely, like blood pressure or cholesterol, because it independently predicts cardiovascular and all-cause mortality. [2]

The good news: unlike your genes, it moves

Genetics set part of your VO₂ max, but training clearly moves it. Pooling many studies, high-intensity interval training raised VO₂ max by a meaningful margin on average, with longer intervals of roughly three to five minutes producing the largest gains. [3]

A practical structure that fits the evidence: build a large, easy aerobic base with longer sessions at a comfortable, conversational effort, and add a smaller dose of hard intervals to push the ceiling. (The popular "Zone 2" label refers to that easy base; the strongest trial evidence for raising VO₂ max specifically comes from the interval work.) [3]

How to apply it

Keep most of your weekly cardio easy enough to hold a conversation, to build the aerobic base, and reserve a smaller amount for hard intervals, such as repeated three-to-five-minute efforts. Consistency over months matters more than any single heroic session. [3]

What the evidence does and does not say

The large mortality study is observational. Fitter people die less, but the data cannot prove that becoming fitter is what extends life, and poor underlying health can itself lower fitness. The case for training rests on combining that association with trials showing fitness is trainable. [1][3]

The useful signal.

  • Higher cardiorespiratory fitness tracks with far lower mortality — up to roughly a fivefold difference between least- and most-fit — with no observed ceiling of benefit.

  • Unlike genetics, VO₂ max is trainable: an easy aerobic base plus regular high-intensity intervals reliably raises it.
  • The mortality data are observational (association, not proof of cause); trainability is what the trials demonstrate.
